Working Together to Prevent Type 2 Diabetes

At Columbus Community Hospital we believe that total wellness is more than just caring for patients when they are sick. In partnership with the National Diabetes Prevention Program, Columbus Community Hospital will offer a Diabetes Prevention Program (DPP), a lifestyle change program proven to prevent or delay Type 2 Diabetes.

CCH’s DPP is a free one-year program for individuals who are at risk for type 2 diabetes that are committed to improving their health.

The Columbus Wellness Center DPP is a one-year program that combines online coaching modules and in person classes with regular check-ins with a primary care physician. Topics covered in the program include:

  • Getting active
  • Food preparation and recipe modification
  • Shopping and cooking to prevent type 2 diabetes
  • Managing stress
  • Coping with triggers
  • Keeping your heart healthy
  • Eating well away from home

  • All program participants must be 18 years of age or older.
  • All program participants must have a body mass index (BMI) of >24kg/m2
  • All program participants must be considered eligible based on either:
    • A recent (within the past year) blood test (may be self-reported) or claim code indicating they have prediabetes, or a history of gestational diabetes mellitus (GDM), according to one of the following specifications.
      • Fasting glucose level of 100 to 125 mg/dl.
      • Plasma glucose level measured 2 hours after a 75 mg glucose load of 140-199 mg/dl
      • An A1c level of 5.7 to 6.4
    • Clinically diagnosed GDM during a previous pregnancy (may be self-reported). A positive screening for prediabetes based on the CDC Prediabetes Screening Test, or high risk for type 2 diabetes on the hardcopy or electronic version of the American Diabetes Association Type Diabetes Risk Test.
  • Participants cannot have a previous diagnosis of type 1 or type 2 diabetes prior to enrollment.

  3. If eligible, our team will contact you with more information, and connect you with a primary care physician to monitor your progress during the program.

*Note: The April 2023 program is currently at capacity. Any inquiries received after March 15, 2023 will be considered for the 2024 program.

Occupational Therapy

Occupational Therapy at Columbus Community Hospital focuses on improving quality of life and independence with daily activities during your hospital stay.  The Occupational Therapy staff combines hands on techniques, appropriate modalities, patient education, and the use of adaptive equipment as needed to maximize function.  The OT team will work with you on activities of daily living such as: bathing, dressing, feeding, and hygiene to ensure that you are performing these skills at your maximal potential prior to being discharged.  The Columbus Community Hospital OT staff includes a Certified Lymphedema Specialist for comprehensive lymphedema treatments which may consist of: manual lymph drainage, bandaging, compressive garments, and skin care.
